WHO WE ARE

Health Equity Rising (HER) is a coalition of community leaders and multi-sector partners working together to end health and social inequities in Sonoma County.

We co-create a healing-centered ecosystem with communities facing systemic harm by

  • Uplifting and centering community leadership and priorities

  • Building a culturally responsive infrastructure through the Community Information Exchange Model

  • Cultivating cross-sector partnerships

WHY WE DO IT

  • In 2022, Health Equity Rising mobilized Sonoma County community leaders and key institutional partners through an 18-month community-planning process resulting in the Sonoma County Agenda for Action. The Agenda for Action (AFA) remains an active, community-led response to the disparities highlighted in the Portrait of Sonoma 2021 Update. The Agenda for Action is an intentional call to leadership throughout Sonoma County to work together to to advance community-led solutions in neighborhoods that have faced the most disinvestment in Sonoma County (Bicentennial, Comstock, Roseland, and Sheppard).

  • Current focus is on two prioritized Calls to Action: 

    • 1. Increase access to high-quality, affordable, multilingual health and well-being services, including preventative, specialized, and complex care provided in multiple modalities (in-person, digital, peer-led, school-based, etc.) without barriers related to immigration status, systems involvement (such as incarceration), and/or ability to pay.

    • 2. Promote community agency and increase access to centralized resources through community hubs in prioritized communities, which are led and staffed by trained community members, and which provide affordable, strengths-based services and supports such as financial literacy, digital literacy, recreation, community-building, information sharing, and resource navigation among other services needed as identified by communities.
