SUPPORT CHANGE EFFORTS IN SONOMA COUNTY

Please consider supporting Health Equity Rising. We support change efforts in the Sonoma County neighborhoods and communities most historically excluded from decision making. These efforts are led by individuals who have developed the Agenda for Action, a blueprint for tackling systemic inequities that have disproportionately affected the county's underserved neighborhoods. They are in the process of developing a roadmap for change to implement prioritized actions that will create lasting systemic change in their communities. Furthermore, we support cross-sector coordination around the delivery of services for Medi-Cal, uninsured, or underinsured clients. Utilizing a Community Information Exchange framework and the Resource Connection Network, a closed-loop referral technology platform, we help Northbay residents get connected to the resources they need, whether it’s housing, food, mental health support, etc.
