AGENDA FOR ACTION

The Agenda for Action is a community-created roadmap that outlines what our communities need to be healthy and thrive. It is also a call to action for leadership throughout Sonoma County to sustainably invest in those most impacted by systemic inequities . Built by listening to the people who are closest to the problem — because those closest to the problem are closest to the solution.

With support from Health Action Together, community members in Bicentennial, Comstock, Roseland, and Sheppard came together to develop this transformative, grassroots, equity-focused Agenda — rooted in data from the Portrait of Sonoma County 2021 Update.

The Portrait measures well-being across three dimensions: health (life expectancy), access to knowledge (educational attainment and school enrollment), and standard of living (income). Together, these make up the Human Development Index (HDI), scored on a scale of 0 to 10. While Sonoma County's overall HDI of 6.19 surpasses both the U.S. (5.33) and California (5.85) averages, scores vary dramatically by race and geography — with several communities, including those named above, scoring at or below 4.00. This community engagement process helped uncover the root causes driving these disparities and identified the investments, policies, and actions needed to reverse them. 

At its core, the Agenda for Action lays out clear priorities — called Calls to Action — and a 5-year implementation plan that helps guide where we focus our energy, resources, and partnerships. It is a shared plan for community members, organizations, and leaders to work together toward a healthier and more equitable community for all.

COMMUNITY ACTION FOR HEALTH EQUITY (CAHE)

A community-led table of grassroots resident leaders from the communities of loving focus,  that Health Equity Rising supports and resources, focused on building the capacity, skills, partnerships, and infrastructure needed for residents to lead and shape the implementation of the AFA within their communities.
