





We are proud to present the Health Equity Rising Strategic Plan. This plan reflects both the urgency of this moment and the long-term vision required to build a more connected, equitable, and responsive ecosystem of care. It builds on years of partnership across health care, social services, community-based organizations, and public agencies, while recognizing that the challenges facing our communities today demand deeper alignment and shared accountability.
This plan reflects more than an organizational roadmap; it is a shared call to action. It affirms our commitment to steward resources responsibly, to deepen collaboration across sectors, and to remain accountable to the community. Together, we will continue building a system where health and social care are not separate, but part of an interconnected and well-coordinated network of care that supports every person in Sonoma County.


IMPACT REPORT
In this report you will find information about our collective work with partners last year which led to:

Strengthened cross-sector relationships and deepened trust with impacted communities

More than half a million dollars in community investment; over $20k invested directly in community leadership; $495k went to community-based organizations and health center partners to support their capacity to deliver culturally responsive services

Successful implementation of closed-loop referrals, enabling real-time visibility into care coordination and service access
-
77 referrals connecting residents to essential care and resources through the Resource Connection Network
-
A 40% increase in participating organizations actively coordinating services and developing a local Community Information Exchange infrastructure to improve care, coordination and support
